About

Pathologic 3 is a psychological horror game where you have only 12 days to save a town from a mysterious plague.
You are Daniil Dankovsky: physician, researcher, prodigy. Your research into the nature of death has brought you to this remote town deep in the eastern steppe, looking for a man said to be immortal.
But the outbreak of a deadly contagion sweeps through the town. The immortal man is dead. You failed to stop it.
Now you must return to the beginning. Fix your mistakes. Save the town.
Every minute counts. Save them all, discover the secrets of immortality... don't go mad in the process.
Bring your medical training to bear. Observation and knowledge are your tools. Work with patients face-to-face.
Examine the sick and make diagnoses. Each patient is a unique story — and a medical mystery. They may lie, omit the truth, or hide their motives. You must separate fact from fiction, and identify the real symptoms. Step by step, you'll get closer to developing a vaccine that could change everything.
Fight for every new day. Ensure the town's survival by imposing your own rules: enforce quarantines, confiscate precious medicine, vaccinate the population, suppress riots, deploy patrols, and declare curfews.
You will make mistakes, face their consequences, and unlock new decrees. Your actions will shape the town—even if the people grow to hate you for it. Power is in your hands. It's up to you whether the streets will see gunfire or fireworks.
Made a mistake? Go back, and fix it.
Time is your tool for navigating a narrative web filled with moral dilemmas and branching paths. Rewind if you think you could have done better. Keep your eye on your ultimate goal—even if the road there is paved with sacrifices.
How far will you go to find the truth? Your mental state directly affects your actions, and even how you interpret events. Falling into apathy or wandering into mania may lead you to new insights... or to your death.

Pathologic 2 felt like a unique, mad experiment of a game, so starkly distinct from anything else in the medium that it would constantly impress us with its creativity. Pathologic 3 is completely different, and that's precisely why it feels the same way. Indeed, it's its contrast with P2 that, to me, really elevates it, and if your love for art does not solely come from the final "product" but from the artistic process and soul behind it, then you'll enjoy recognising all the creative decisions made by the folks at IPL in the effort of presenting the same world via a wholly alternate perspective. P3 will invoke different emotions and challenge you in different ways to mirror Daniil's view of the Town: for example, the Haruspex's actions in P2 felt driven by a sense of love and compassion, shared by Artemy and the player; the Bachelor has some of that, sure, but his love feels more... detached. With the patients he inspects, both he and the player take on the cold demeanour of a doctor who cannot afford to get affectionate, as he knows their fate is sealed regardless. And by that lens, it's no big deal if you make a mistake, right? After all, you can just go back. The most valuable resource in both P2 and P3 is time, but in P3 time is liquid and may be held in your grasp. Your planning thus no longer requires you to ask yourself "Do I have the resources to reach this district?" but rather "Do I have the resources to reach this day?". This is a big difference, as you can now erase mistakes, but the results of doing so may be more mistakes to fix at another point in time. I find the concept great, though I admit the economy feels a bit off: it did not evoke the feeling of "I must make a hard choice" but rather "Oof, I might have to grind a bit later". It was a small issue for my taste though, and the time travel mechanic itself is key to the game's ludonarrative balance (maybe a snobbish term, but it is a genuinely important game design aspect to me, and one that IPL handles so gracefully). When I think of ludonarrative, I think of at least two elements: on one hand, it's about how well the mechanics tie into the story, and in this case time travel is not just a gameplay gimmick but a fundamental facet of the game's narrative and metanarrative that becomes clearer and clearer as you progress; on the other hand, it's about how well you can twist and turn the player's emotions to reflect those experienced by the protagonist. In P2, these were primarily tension and stress, as a wrong step could cost a lot to the player and to the Haruspex; there is a bit of that in P3, but a more notable one is confusion: just as Daniil is lost in time in a remote town, so too is the player, as they are constantly second guessing how their actions will affect future, present and even past in the tangled web of the game's story, and as they try to piece it together. P3 is all about piecing together fragmented clues, and the detective work you perform reflects Daniil's strive to uncover the mysteries of the town, of the plague, and of Simon Kain. This investigating is also exemplified in the daily patient diagnoses (which, again, are not just a gimmick minigame but tie into the narrative). I found them really well made. Of note is how important it is to listen to the patients, and dialogue in general in this game. P3, more than P2 imo, places special emphasis on dialogue: multiple characters can talk at once, wrong answers can have major consequences (but IPL gets away with this since they gave you the option of undoing mistakes), and more things I won't spoil. Indeed, being able to repeat days means you'll be able to see much more of the narrative compared to P2, and I find the narrative in P3 to be an absolute triumph. Pathologic 3's most memorable moments come from its story. It reminds me a bit of when I touched Deionarra's sensory stone in Planescape: Torment: incredibly poignant scenes made possible by the game's surrealist setting and the protagonist's unusual abilities. It's also helped by Daniil being a really great character, and being respected as an authority and by other authorities was a lot of fun. His quest, in particular, feels orthogonal to the game's events at first yet everything ends up revolving around his goal to overcome death, and the setting and the themes already explored in P2 become a launch pad for something more. That said, there were fewer memorable "emergent" moments, i.e. stemming from gameplay mechanics, unlike P2 where you'd remember the narrow victories over your hunger and exhaustion meters or the nightly gamble of characters living or dying. There are also mechanics in the game that I don't think work perfectly. The Shabnak is probably the worst offender: infected districts in P2 where stomach-churningly terrifying, whereas in P3 they're just kinda annoying (music was dope though). But I think it's just the gameplay not being built around this kind of mechanics. Also, while I am ok with not having an open world (I loved it in P2, but I'm fine doing without it this time), I do feel that travel in general is not that interesting, despite a lot of time being spent on it. How does one make walking interesting? In P2 they did it by giving you limited resources, while in P3 they didn't. The mania/apathy system is alright, but kicking things around is sorta goofy and in general the town feels like set dressing rather than an interactive being. Finally, I really love how the decree system is used to "unlock" more days, but after that it's mostly just flavour. Whether pros or cons affect your enjoyment of the game more depends on your subjective taste, but to me it's an S-tier game through and through (P2 may have been S+ though :D ). I can't imagine how much fun the IPL designers and writers must be having in thinking of Pathologic 4 (and how much dread their programmers in implementing it). If it comes out just as good, then P2+3+4 might end up being my favourite trilogy of all time. In IPL's shoes, I would consider leaning more on horror next time around; I'd consider exploring more the role of grief (for a game all about people dying, I've seen very little crying); if it still revolves around walking, I'd try making that as engaging as possible (how's your twin sister making it hard for you?); and I might put even more emphasis in dialogues (we know now that the interlocutor's mental state affects Clara; and besides, we know her words to be really powerful). Grant us changeling route, grant us changeling route, plant changeling route on our brains to cleanse us of our beastly idiocy.

The game has fundamental issues which cannot be patched out and the translation is very bad. It's the worst game in the trilogy.
Pretty uninteresting tbh. I don't like the scrambled structure and it couldn't really hook me to play more. I enjoyed the RPG elements of the previous games a lot more. Also not a fan of the UI at all.
daniil dankovsky's terrible horrible no good very bad steppe vacation and subsequent police interrogation. this man doesn't eat or drink for 12 days save for drinking morphine and eating straight up coffee beans, of course he's fighting death anyone would be with that diet. 10/10 masterpiece of a game
One of the most fascinating games since... well Pathologic 2 I suppose. I recommend this game to basically everyone who loves video games. I am flabbergasted how Ice Pick Lodge managed to return to the Gorgkhon for a third time with the same characters, the same world, the same overall plot, and again create something fresh, beautiful and unique. Compared to Pathologic 1 and 2 this is less of a survival game. The (mechanical) gameplay difficulty is far more lenient than the first two games, to the point that I would call this game rather (mechanically) easy. As such I would particularly recommend this game to people who have been interested in Pathologic but have felt daunted by the notorious difficulty and/or jank of the first two games. However, I would also say two more things about this game's difficulty: 1. The mechanical difficulty doesn't matter as much 2. The difficulty stems more from what you make it in your own head. What do I mean by 2? Well, this is basically a Groundhog-Day-Style narrative where you can repeat sections, thereby influencing the future, thereby influencing where the narrative goes. This leads to an ever-growing network of dependencies between events, characters and outcomes, which is difficult to manage at times, however, this feels rather purposeful. Through these time travelling mechanics, the world and actors of Pathologic 3 can be explored in such depths that they will stick with you for long after you're done playing the game and it was difficult for me to let go (and I feel tempted to play the first, second and replay the third all over again immediately). I don't think I can say much more without going into heavy spoilers, but please, give Ice Pick Lodge your money because (if the original plan is still on their plate) there SHOULD be a Pathologic 4 somewhere on the horizon that focusses on a different character of this cadre and this studio, this world deserves to be explored to its fullest (if the devs want to).
