From the hardworking Prole to the affluent Banker, each fate offers unique abilities, challenges, and ways to interact with the city and its inhabitants. Your choice defines your journey:
Align with the Cortex to stabilize and protect the city.
Join the Scumbags or Zealots to spread the Rot and sow chaos.
Remain Neutral and immerse yourself in daily life, working in factories or exploring the city.
An ever-present threat, the Rot visibly alters the city as it spreads. Roles like Liquidators and Scumbags are pivotal in either containing or exploiting this menace. If left unchecked, the Rot will consume everything, leading to the city's demise.
ANEURISM IV is a sandbox simulator first and foremost. The city reacts dynamically to player actions:
Take Down the City: Collaborate with others to bring about the city's downfall and witness its reset.
Protect the City: Work to maintain order and keep the Rot at bay.
Live Your Life: Engage in everyday activities or carve out a unique path—it's your choice.
Survival depends on your willingness to work. Earn credits by taking on various jobs—labour in factories, manage resources as a Prole, or manipulate finances as a Banker. Looking at this steam page right now (2026-08-02), the recent reviews are sitting at "Mixed." People are complaining about this and that. People are saying the game is dead. Let me tell you the truth: Most of them are entirely missing the point. I follow the [b]Dollar per Hour Rule[/b]: where every dollar of the cost of a game should equal at least one hour spent playing that game. (e.g., 80 dollars for at least 80 hours of gameplay, or it was never worth your time or money) I paid $13 CAD for this. I'm at 120 hours. It passed the test a long time ago, and I'm still going. [h2]The Setup[/h2] At first glance, it draws you in with this beautiful, crazy, dark, near-psychedelic aesthetic. It looks like one of the other newer experimental indie immersive-sims, like Cruelty Squad or Dread Delusion. But then you realize this one has something they don't: every single character in this world is a real player. It shocked me at first. When I first walked down a busy street, and saw everything going on. After realizing that, and playing for a bit, I started seeing similarities between ANIV and some DarkRP servers from Garry's Mod, but this game was running completely wild with [b]zero[/b] admin or moderator intervention. Just raw, unmoderated chaos, and to my surprise somehow actually flowing [i]nearly[/i] perfectly despite that. [h2]Another Game in the Game[/h2] But here is the super interesting part for me. After spending 120 hours inside, I realized this isn't truly a plain RP server-like game at all. behind the facade is a persistent, multiplayer, hidden PvP-fps game happening right under everyone's noses, driven by strategic, guerilla-warefare in a attackers vs defenders way. Look at how the factions actually operate: [list] [*][b]The Scum:[/b] These guys are literally playing a team-based extraction shooter. They push up to the surface, scavenge supplies and gear, destroy government property for spiritual currency, and then extract back underground to gear up for the next raid. (or life) [*][b]The Cortex:[/b] These players exert the will of the mysterious totalitarian goverment. They are playing a completely different game. They act as an ever-watching defense force, trying to maintain order with an unruly populous while fighting off the Scum's constant Guerilla attacks on them, and government assets. [*][b]The Citizens:[/b] Everyone else. Caught in the crossfire, just trying to survive and maintain their own agendas while this shadow-war happens around them. [/list] It's truly groundbreaking how well it's all pulled off. [h2]The Real Problem[/h2] Now, is it perfect? No. The people complaining in the reviews bring up a lot of valid points, but what most of the reviews don't tell you is at this point I've seen that the devs are actively patching and fixing a lot of the issues right now, if they haven't already recently. Despite that, one review mentions that the game should truly still be in early access, and they're entirely correct. The game is great as it is, and very fun, but it feels as though the game itself [b][i]yearns[/i][/b] to be worked on, and expanded still. The [b]actual biggest problem[/b] with this game is the population. [b]It’s not dead but it's definitely on life support.[/b] When you log in, you can see that clearly when you realize there are dozens of empty servers and only one at a time that is decently populated. [h2]My Verdict[/h2] I talked to a lot of players inside game, and I have a theory as to why the reviews are so mixed. This game suffers from a rare curse: the player base actually loves the game so much, and they are so obsessed with what it [i]could be[/i], that they have become hyper-critical. They are review-bombing something they actually care about. That doesn't apply to all the reviews, and there are definitely many sincere greivences, but don't be driven away by them. [h2]Conclusion[/h2] [i]Aneurism IV[/i] is a diamond in the rough. The 120 hours I have logged prove the core loop works. Buy the game, learn the hidden mechanics, and help rebuild the server population.
Cortex players will run in front of you during wartime so that you accidentally hit them with your screwdriver and thus can be limited, putting You a WHOPPING FIVE WHOLE ANEMNECYTES IN DEBT which can translate to a real time afternoon in the labor camp (at negative five anemnecytes you basically have to sit in time-out until the server resets (unless you actually have the time to roll boulders for an hour with intentionally irritating HUD and mechanics), this also reduces a hidden stat called karma at a rate of TWENTY (TWENTY) nearly locking you out of the limitator role. Also it is incredibly complicated with no tutorial whatsoever (not even basic controls) (imagine playing modded Terraria without the wiki)
This game really doesn't deserve the wrap it get's nor do it's developers, for those who are willing to try something fresh please I implore you keep this game alive.
