You are Daniil Dankovsky: physician, researcher, prodigy. Your research into the nature of death has brought you to this remote town deep in the eastern steppe, looking for a man said to be immortal.
But the outbreak of a deadly contagion sweeps through the town. The immortal man is dead. You failed to stop it.
Now you must return to the beginning. Fix your mistakes. Save the town.
Every minute counts. Save them all, discover the secrets of immortality... don't go mad in the process.
Bring your medical training to bear. Observation and knowledge are your tools. Work with patients face-to-face.
Examine the sick and make diagnoses. Each patient is a unique story — and a medical mystery. They may lie, omit the truth, or hide their motives. You must separate fact from fiction, and identify the real symptoms. Step by step, you'll get closer to developing a vaccine that could change everything.
Fight for every new day. Ensure the town's survival by imposing your own rules: enforce quarantines, confiscate precious medicine, vaccinate the population, suppress riots, deploy patrols, and declare curfews.
You will make mistakes, face their consequences, and unlock new decrees. Your actions will shape the town—even if the people grow to hate you for it. Power is in your hands. It's up to you whether the streets will see gunfire or fireworks.

I am extremely impressed at how this manages to instill the same frantic frustration that was one part of what made the previous Pathologics so potent, but with generous resources and an abundance of resources. It puts so much on your plate that if you played it straight through once like P2, you would likely no even get half of everything done. The prose did feel less sharp though. I am trying to find the perfect words for why but I think it boils down to this: In Pathologic 2 people spoke mystically but in a way that often circled back to grounded connection. In Pathologic 3, people speak mystically but often connecting it to further mysticism. It is a natural consequence in the POV shift (and also present in Pathologic 1) but it led to me really only growing attached to Eva. I really like the medical inspections. I can definitly imagine a different game with them at the forefront. It felt like required guesswork sometimes, and I'd like to see it a little more robust but it was a really cool part of each in-game day. (Spoilers Below) [Spoiler] I got the default You Did Everything You Could but destined failure "Rot" ending on my first go and then went back to get the True Immortality ending as well as saving Lara because im still attached to her from the second game. Despite the whole game being centered around time traveling around, doing it after reaching credits feels bad. Even when it is totally narratively woven it, it still feels like cheap achievement hunting. [/spoiler]

Pathologic 3 has broken my concept of perception and my understanding of Death. So instead of writing a review I'm just going to list things I like in now ordered like a shattered mirror. In Pathologic 3 you are a suicidal Doctor that keeps from eating a bullet by kicking bins playing on playground equipment and buying hard drugs from children. This game has the best save system I've ever scene Rats are ♥♥♥♥♥♥♥♥ kill them at every opportunity There are bugs that will have you pulling out your hair most can be solved by repeating quets use ctrl + shift + ` to access console command use restore_timelife when this happens Pathologic 3 is one of the most unique games I've ever played and I thoroughly recommend it.
This is the first Pathologic game I've ever played and I enjoyed it a lot. I was always eager to return to the game and keep learning more about the town, its characters and secrets. I also liked Daniil's character and the way he approached the world: with skepticism and a knowledge of science.
5/5 mania simulator.
