Pathologic 2 is a groundbreaking open-world horror RPG. Resist the plague. Make medicine. Heal people. Perform an autopsy. Trade to get what you need. Fight and kill if necessary. Survive. Struggle with an outbreak in a secluded rural town that is rapidly turning into hell.

Reviews
Pathologic 2 is a frustrating, disturbing, and often depressing experience, and it's fantastic! Taking place over 12 days as a small town succumbs to a deadly plague, Ice-Pick Lodge creates a game that makes you feel just as helpless as its characters. Whether it's scrambling for enough food scraps to survive another hour or fighting your way through blood-soaked streets to reach another save point, Pathologic 2 is pure survival. Yet, despite how bleak and difficult the game can get, its gameplay loop becomes surprisingly addictive. Granted, the survival mechanics can occasionally clash with the more story-driven sections, particularly when cutscenes interrupt the flow of gameplay. This is most noticeable during the opening and later days. Still, no matter how miserable it can make you feel, Pathologic 2 remains a challenging but ultimately rewarding experience, with a strange ability to make you want to return for more punishment.
This game as well as it's successor doesn't cost time and money invested in it.

For scale, across Steam · Top Sellers · Poland over the last 30 days: the 21 chart moves that followed a price cut of more than 20% went a median of 12 places towards #1, and the 2,501 moves with no price cut behind them went 1 away. One store, one chart, one country — it is the only place we have enough observations to say even that.
Prices outside the United States have been collected since 6 August 2026 and chart positions since 19 July 2026, so a cut older than that has no market to compare and a market that joined later has no history yet. Nothing here is converted between currencies.

I wouldn't say it's better than the original, but I have a feeling it's a bit more forgiving. That said, it doesn't mean Pathologic 2 doesn't pack a punch at times. You will find yourself annoyed, sprinting from one side of the map to another to try tie up all the loose ends before the end of the day, struggling with resources, and all in all just struggling in general. But the town is alive, and so are its inhabitants. The developers clearly did everything they could to fully immerse you in the town on Ghorkon, making you feel like a cog in a machine and a crucial part of the community. The game never stops guilttripping you for your mistakes and doesn't reward you for your successes, but surviving Pathologic 2 is a success in itself. Caring for the children and seeing the protagonist's fatherly relationship with them is rewarding enough. Curing your patients, or waking up and seeing that your efforts didn't go to naught and nobody had fallen ill in the night, is rewarding enough. I have my own opinions about the handling of the story, but go and play Classic to form your own. The combat still sucks and is quite annoying. Here's a tip, unbind sticky keys and spam shift, there's no use fighting anyone. It's too annoying. Also why did they de-twinkify Daniil
I legit tried this game twice before beating it, both playthroughs were years apart and both playthroughs were equally confusing and frustrating. This time I actually beat the game but I got the worst ending...lowkey wasn't feeling like getting a real one so I'm just gonna leave it at that. Regardless, story and gameplay was fun enough and interesting to continue, even when I save scummed a numerous amount of times.
