Big Bad Pharma is a cosy first-person pharmacy-stocking sim with a wicked streak. On the surface it's everything you want from a tidy little restocking game: neat shelves, a quiet shop to run, and that deeply pleasing click of the last box sliding into place. Underneath, it's a cheeky satire of corporate healthcare, and the further up the ladder you climb, the more you might want to ask yourself whether it was worth it.

Every shift starts the same way: shelves stripped bare, product everywhere, and you.
Every single one of the 12,792 items on that floor has a home. A specific shelf, in a specific bay, in a specific aisle, and the shop isn't finished until every last one of them is in it. Pick things up, carry an armful, and slot them into place. Categories run from digestive relief to migraine remedies to a genuinely suspicious quantity of allergy medication, all of it wearing a label that's almost a brand you'd recognise. Legally speaking.

Sort products into the right aisles, from digestive relief to migraine remedies to a suspicious number of allergy medications. Nobody climbs the corporate ladder on foot forever.
Behind the staff door there's a beige desktop PC running an operating system that time forgot. That's the Rx Upgrade Hub, and it's where your wages go. Carry more in one trip. Move faster between aisles. Jump higher, throw further, and unlock the four tools that turn a long shift into a short one:
Locate: light up every stray item of a category, wherever it's hiding
Auto Shelve: send what's in your hands straight to where it belongs
Magnet: pull nearby product to you without bending down once
Sweep: gather up a whole patch of floor in a single pass
Max everything out and you become a truly well-rounded (and slightly terrifying) employee of the month.

There's more to this pharmacy than restocking. Twenty-one achievements trace your journey from bleary-eyed newcomer to something a little more... corporate. There are things written on mirrors. There's a radio out back with four decades of music on it. There's a toilet, and yes, it flushes.
Clock out at the end of a shift and the numbers come up: items shelved, sections completed, time on the floor, and a running record of every shift you've ever worked. Then the shop refills, the day ticks over, and it all starts again, which is either deeply comforting or the entire joke, depending on how you're feeling.

Big Bad Pharma is warm, funny, satisfying to play, and just self-aware enough to make you smirk. It's a game about small satisfactions and big questions, made by one person with a sharp sense of humour and far too much time spent thinking about shelf geometry.

All 3D assets are my own (hence why I kept it very basic). All textures and decals in game are made by me (you may see where I've struggled in places). You might think: oh my god, more than 12k small scattered items? Idk man... That was me, too. BUT, I'm glad I got this game. It's super smooth, you get some sort of space to choose what to place where inside specific categories. Finding out what goes where isn't too hard either once you get into it a little. Nice background music, occasional howls. :D Perfect game to actually work on in shifts until you complete it.

Thank you for making this game!!!! It soothes my mind so much that I feel like I'm sleeping better after playing. And THANK YOU a billion times for removing all the AI artwork and replacing it with hand-drawn artwork, it's so perfect! P. S. to anyone who saw the review that said there wasn't enough room for all the items...there is exactly enough space for all of the items.
Really solid game, especially with the new updates and fixes added to it, making it much more enjoyable. It's more challenging compared to similar games, the abilities/skills definitely help but they were not made to speed up the process by 300% like in other games, but rather a 25%, so you definitely need some patience, also because there's so many items that look exactly the same but you won't know what they are until you pick them up and the color coding only takes you so far, takes a while to get familiar with all of them. New game with lots to improve but super enjoyable nonetheless, solid 9/10! ^^
I highly recommend Big Bad Pharma!! Sophie has put a lot of love in to her game and it really shows. I've only been playing for an hour and I'm really enjoying :D 5/5 stars!!

Sadly probably gonna refund this one, which is disappointing because I was really excited for it. I think it should've been released as early access instead of full release: there is a lot of quality-of-life missing and no accessibility settings (the head bobbing is really intense, and if you're colourblind there is no way for you to differentiate different types of medications without picking them up), there is no indication or explanation that you can purchase upgrades at the front computer, and the item-to-shelf ratio is deeply unsatisfying. In a game all about organising, and with an alphabetical order on top of that, it doesn't make any sense for meds not to fit properly on the shelves. There is no way of knowing if a given medication will take one, two, or more spots on a shelf. On the other hand, the graphics are a huge positive. Everything seems to be hand-designed and the aesthetics are very satisfying. But that only makes it even more baffling why you'd choose to put nauseatingly ugly AI slop on the achievements, where they assault your eyes each time you're considering booting up the game.
I don't write a lot of reviews, but This one feels like it needs one. It's a great addition to the "organize all the things" genre, and there are things it does that others don't that i wanted to call out. For one, this game tells you how many items go into the "slot" you're filling, which is super helpful and not all games do. But the OTHER thing it does which I love love love is that it tells you how many "slots" each item gets. So instead of constantly having to rearrange my shelves to put all of the Kleenox together, i knew from the start how many i would have and could plan my shelves accordingly. I also love that you also have access to a master list of every item and how many there are of each. That made it that much easier to do a more master plan of my store. So super handy. I will say I maxed out all the skills and didn't see anything like "sweep" or "locate" among them, now did i find the mentioned 'back alley,' maybe that shows up on a future playthrough? i'm not quite ready to do another one yet haha, but i really enjoyed this one! Almost 13k items sounds like a lot and it is, but it's enough to feel engaged and not so many that it feels overwhelming. I really didn't have any problems differentiating items too much, even if some of the boxes look the same, you also have a list of everything you're holding which has the name of the item and the symbol of what category it belongs to (no guessing!). I also kind of loved that it had both OTC and behind the counter items, it felt more like a real store. I played on Steamdeck, and it played pretty well. Some of the text was a little small, and the buttons were a little touch-and-go, but the touchscreen also works which helped with things like using the ability upgrade computer (it's almost impossible to navigate with buttons). At any rate, great game, and i'll def be giving it another playthrough at another time.
Nice sorting game. Multiple colour coded clues for placement, easy to suss what goes where, despite a tremendous amount of items to put back on shelves. I really enjoy the artwork for this game. I remember glancing at this game with the old AI art and passing it up. Then I saw it again with the new cover art and never clicked buy so fast in my life. I love that genuine art vibe thing. I'm glad they've replaced all the AI stuff with hand (mouse) drawn art instead. Bonus points for letting me relive Windows XP.
This game is so solid! It has come a long way since release and it hasn't broken it's first week yet. You can tell the dev cares about their game and what the fans think about it. I can imagine they are working tirelessly to make hotfixes. IT IS SEEN AND APPRECIATED! The game was good before and even better with the QOL improvements. There are def some bugs but the fun and potential is so there. *I would like to edit that I am playing this on steamdeck and may have additional bugs not seen on pc*
